Ms. Green, 49, an immigrant from Jamaica and devout churchgoer, had been admitted to the hospital’s psychiatric ward on June 18. Twenty-four hours later, having yet to be seen by hospital staff, she collapsed onto a waiting room floor, where she lay, ignored, for nearly an hour. She died soon after.
